Submit A Claim Online. It’s easy. Here’s how …

To submit your claim online, please take the following steps:

  1. Click on the link or the desired form under the respective Product.
  2. Complete all mandatory fields and upload images of the required supporting documents indicated in the list below.
  3. Once completed, scroll to the bottom of the form, check the Declaration box if you are in agreement and click “Submit”.

Once the form is submitted, you will receive an acknowledgement e-mail confirming receipt.

Please note that even though a claim is submitted online, the applicant’s official signature and the original supporting documents must be submitted in order to further process the claim.

  • Statement of Accident
  • Estimate of Repairs
  • VAT letter/BIR #
  • Recent Certified Copy
  • Letter confirming coverage from your Insurer
  • Copy of Driver’s Permit (Both Sides)

Windshield Requirements

  • Completed Motor Claim Form
  • Certificate of Insurance
  • Driver’s Permit
  • Certified Copy
  • Estimates/ Bills/ Receipts
  • Photos of Damage must show number plate
